Go to the Wilkin County Recorder's Office in Breckenridge in person and bring a valid photo ID, the full name of the person who died, the date of death, and payment. For certified copies, you must meet the eligibility requirements under Minnesota Statute 144.225. If you do not qualify, you can ask for a noncertified copy or show a legal reason for needing a certified one.

Mail requests can go to the county recorder or to MDH. For MDH, download the application form at health.state.mn.us/people/vitalrecords/docs/dcappia.pdf. The form must be notarized before mailing. Certified and Noncertified Copies

Minnesota death records are issued in two types. You need to know the difference before you apply.

Certified copies carry the official state seal and are accepted by courts, banks, insurance companies, and government agencies. They are required for estate settlement, insurance claims, property transfers, and other legal processes. The first certified copy costs $13. Additional copies of the same record ordered at the same time are $6 each. Eligibility under Minn. Stat. 144.225 applies.

Noncertified copies are available to anyone. They cost $13 and include the same information as the certified version but do not carry the official seal. They are not accepted for legal or financial purposes but work well for genealogy and personal research.

For more on who qualifies and what documents to include with your request, see the MDH eligibility page at health.state.mn.us/people/vitalrecords/tangible.html.

Deaths before 1908 are not in the MDH registry. For older records, you need to look at county files or historical archives.
